The Monthly check-in is a quick questionnaire that you complete once a month to assess how your condition is affecting your ability to do daily activities.
What is the Monthly check-in?
The Monthly check-in is a short set of questions that assesses your physical, mental, and emotional health. It takes about 5–10 minutes to complete and provides a score to help you better understand your functional capacity.
Functional capacity refers to your ability to perform tasks and activities that are necessary or meaningful in your daily life.
For people with energy-limiting conditions, a reduced functional capacity can be one of the most debilitating aspects of daily life. It affects how much you can do and how consistently you can do it. Until now, the impact of activities has been difficult to measure or communicate, but the FUNCAP27 questionnaire is designed to change that.
Tracking symptoms alone doesn’t always reflect true progress. For many people with an energy-limiting illness, we may be able to do more while still tolerating a similar level of symptom severity as our condition improves. That’s why tracking functional capacity alongside symptoms offers a more complete and realistic picture of your health over time.
How does the Monthly check-in FUNCAP work?
FUNCAP27 is a scientifically validated, patient-informed questionnaire with 27 questions that measure your functional capacity.
The questionnaire covers 27 activities across 8 areas, and for each activity asks:
What are the consequences for you if you perform this activity?
To what degree does this limit your ability to do additional activities?
The questionnaire covers the following categories:
Personal hygiene / basic functions
Walking – moving around
Being upright
Activities in the home
Communication
Activities outside your home
Reactions to light and sound
Concentration
For each item in those categories, you’ll be asked to rate the impact using the following scale:
0 – I cannot do this
1 – My capacity will be severely reduced for at least three days
2 – I can do little else on the same day and for one to two days afterward
3 – I can do little else on the same day
4 – I must limit other activities on the same day
5 – This rarely affects other activities
6 – Unproblematic – does not affect other activities
What do the Monthly check-in scores mean?
After completing the questionnaire, you will be given a score out of 6.
A score between 5.8 and 6 is typical for individuals living without a long-term health condition or disability.
Lower scores reflect reduced functional capacity.
Over time, your scores and patterns can help you better understand how your condition is changing and guide conversations with your care team.
Why does the Monthly check-in matter?
Being able to explain your functional capacity—what you can and can’t do day to day—helps you and those close to you understand how your condition affects your daily life. It offers a clearer picture of your overall health than symptoms alone. By measuring it each month, you can track progress and spot patterns in how well you're managing daily activities over time.
How should I answer the Monthly check-in questions?
Try to answer based on your average experience over the past month, not just your best or worst days. If a question involves an activity you haven't performed, estimate how it would have affected you based on similar experiences.
For activities like going to the shop, consider the entire process, including preparation (such as getting dressed), travel, the activity itself, and the return journey. Score these activities while keeping in mind all the steps involved.
We recognize that not every question will feel relevant or perfectly suited to everyone's circumstances, but each response helps build a clearer picture of your functional capacity over time.
Where can I do the Monthly check-in?
The Monthly check-in will be available in your today's tasks. After you complete it, the check-in will be available again in four weeks.
Can I pause or edit my Monthly check-in responses?
You won’t be able to edit your answers after submitting, so take your time and review them before finishing. You can take breaks during this assessment, and your progress will be saved, allowing you to resume at any time.
Where can I find my Monthly check-in past results?
At the end of your assessment, you'll receive a score and a detailed breakdown of the areas most impacted. To compare and view previous scores, navigate to your Profile page and tap Monthly check-in. This will allow you to track your progress over time and identify trends in your health and well-being.
